*We realize that the on-line bankruptcy form, in particular, is extremely thorough and asks for a great deal of information. The program will allow you to save your work and come back to it later. You do not need to complete this form prior to your initial consultation; however, all of this information will be required by the bankruptcy court and your form must be complete before we can file a bankruptcy case on your behalf. Before filing your bankruptcy case we will also need copies of your tax returns or W-2s for this year (if available) and your tax returns for the last two years before this one; as well as copies of all of your pay stubs for the past six months.
